Option 1 - Standardized department verification of training form
Cost: $60
Standardized department verification of training form modeled on National Association of Medical Staffing Services best practices. Form information includes trainee full name and date of birth; training specialty; dates of training; program completion status; program accreditation; any relevant leave, remediation, and/or early dismissal from the program; and any additional comments, as necessary.
Note: For fellowships, we do not offer a standardized department verification of training form. Only the unique institutional form (option 2) will be completed upon payment and request.
Option 2 - unique institutional form
Cost: $105
The program will complete a unique institutional form upon request. Please send completed, signed forms to urores@uw.edu
Verification Payment
The “invoice number” should state the full name of the trainee for the training verification
Estimated completion: 10 business days from confirmation of payment and receipt of release agreement
